主旨：本校111學年度第1學期日間學制學生學習成果導向問卷網路填答活動，將於111年12月21日（星期三）上午10:00開始至112年1月9日（星期一）中午12:00止，敬請本校學生配合辦理。

Subject: The University’s Student Learning Outcome - Oriented Questionnaire online response activity for the 1^st semester of SY 2022 will start at 10:00 a.m. on December 21, 2022 (Wednesday) through 12:00 noon on January 9, 2023 (Monday). We appreciate your cooperation.

說明：

Description:

一、依據「國立臺北教育大學教學意見回饋與支持系統實施要點」辦理。

1. Implemented on the basis of the “National Taipei University of Education Implementation Guidelines for Instructional Suggestion Feedback and Support System”.

二、本校每學期期末針對學士班及碩、博士班所有課程（服務學習課程除外），實施學生學習成果導向問卷之填寫；問卷部分題目會因教師之勾選而有不同。學生填答完全部學生學習成果導向問卷後，方得參加111學年度第2學期第一階段登記選課。

2. At the end of every semester, the university will ask students from all undergraduate and graduate school programs to participate in filling out the Student Learning Outcome - Oriented Questionnaire (except service learning courses). Some of the questionnaire questions may be different depending on the instructor chosen. After the student completes the questionnaire, he or she is then allowed to choose courses during stage 1 of the course selection for the 2^nd semester of SY 2022. 

三、問卷旨在瞭解同學對教師教學與自身學習狀況的看法，並據以改進教學品質，提升學生學習成效。因此，問卷「綜合意見」的部份，歡迎同學能提出具體和建設性的意見。

3. The purpose of the questionnaire is to understand how students view the instructor’s instruction as well as their own learning situation. This enables instructors to enhance the quality of their teaching and improve the learning results of their students. Consequently, for the “comprehensive feedback” portion of the questionnaire, students are very welcome to present constructive suggestions.  

四、本問卷填寫系統於學生填答後，只能統計調查結果，無法查詢個別學生填答資料，可以確保填寫學生的隱私，請同學安心據實回答。

4. After the students answer the questionnaire, the survey system can only collate the survey results; it cannot look up the response information of individual students. This guarantees the privacy of the students. Please feel at ease and answer the questionnaire truthfully. 

五、為鼓勵同學提供寶貴意見，主辦單位將於活動結束後，抽出50位填答同學，贈以300元等值為獎品。

5. To encourage students to provide valuable feedback, the organizers will pick 50 students who had filled out the questionnaire and award them with a NT300   gift after the activity is done.  

六、請由本校首頁—熱門連結—iNTUE校務系統(新）登入個人學號及密碼後，點選「學生專區—教務資訊—線上選課—學生學習成果導向問卷」。
